Commercial drivers (and non-CDL drivers that operate a commercial vehicle) are required to have a medical certificate.
Dr. Carmine Gangemi is a Certified Medical Examiner and can perform your DOT Medical Examination. His office is located at 369 W. Blackwell Street, Unit 2 (in the Doctor’s Park across the street from St. Clair’s Hospital) in Dover, NJ. Examinations are available by appointment to accommodate your schedule (including weekends). Please note that the exam takes about 45 minutes to perform and costs $95. If you have any questions, feel free to contact Dr. Gangemi.

Reminders Before Taking the Medical Exam:
- If you wear glasses or contact lenses when you drive, please make sure that you bring them with you to the exam.
- If you wear a hearing aid, please make sure that you bring it for the exam.
- When you arrive, you will be asked to provide a urine sample as part of the exam (not for drug testing). Your blood pressure will be measured. Try to avoid caffeine for at least two hours before the exam since it tends to increase blood pressure. Nicotine also has the potential to raise your blood pressure. If you have been prescribed blood pressure medication, it’s important to take it as prescribed (don’t change the dosage).
- If you use a CPAP or similar device, please be able to show compliance (a copy of the CPAP compliance report) regarding its use.
- If you use insulin, please have all necessary paperwork from the doctor prescribing it and monitoring your condition. The specific form is MCSA-5870. This form is available on the FMCSA website, and the doctor prescribing the insulin must fill it out.
- You will be asked to list any prescribed medications as well as the contact information of the doctor who prescribed those medicines (it’s helpful to prepare a list ahead of time).
- If you have been granted a skill performance evaluation certificate, please bring it to the exam. If you have been granted an exemption for hearing, vision, or seizures, please bring proof of that waiver with you.
- You will need to present a form of photo ID to verify your identification.
- Relax during the exam. The medical exam is an objective process to collect information that establishes your “fitness for duty.”
